I, the undersigned parent/legal guardian, give permission for my child(ren) to ride the church bus provided by Calvary Baptist Church for Vacation Bible School (VBS) transportation. This includes daily pick-up and drop-off during the week of VBS. I understand that every effort will be made to ensure the safety of my child(ren) while being transported. I acknowledge that participation in any transportation involves a certain level of risk. I voluntarily assume all such risks, including any injury, accident, or other liability that may occur during transport to or from VBS. By signing this form, I agree to release, indemnify, and hold harmless Calvary Baptist Church, its staff, volunteers, drivers, and representatives from any and all claims, liabilities, or causes of action that may arise as a result of my child(ren)'s participation in this transportation service. I understand that seatbelts (if equipped), safety rules, and proper behavior are expected at all times while riding the church vehicle, and that Calvary Baptist Church reserves the right to deny transportation if these expectations are not met.
I understand that during Vacation Bible School (VBS) at Calvary Baptist Church, photos and videos may be taken of the children participating in various activities throughout the week. By signing below, I give permission for Calvary Baptist Church to take photographs and/or video recordings of my child during VBS. I also grant permission for these images and/or recordings to be used in church publications, printed materials, church presentations, and social media platforms such as Facebook, Instagram, or the church website. I acknowledge that no names or personal information of my child will be shared publicly without additional consent. I release Calvary Baptist Church from any expectation of confidentiality and from any liability for the use of my child’s image in accordance with this release.
By signing below, I, the parent/legal guardian of the participant, hereby release and hold harmless Calvary Baptist Church of Statham, its staff, volunteers, and representatives from any and all liability for any injury, illness, or accident that may occur during Vacation Bible School (VBS) activities. I also acknowledge and consent to my child receiving instruction in the Christian faith in accordance with the doctrines and beliefs of Calvary Baptist Church. Furthermore, I give permission for my child to be transported to and from VBS activities by the church bus or authorized church vehicles. I understand that all reasonable precautions will be taken for the safety and well-being of my child, and I assume all risks associated with participation. I have read and understood this release and consent form and voluntarily agree to its terms.
